The Massachusetts Department of Public Health (DPH) seeks proposals to implement the Prevention and Wellness Trust Fund (PWTF) of Chapter 224 of the Acts of 2012: An Act of Improving the Quality of Health Care and Reducing Costs through Increased Transparency, Efficiency and Innovation. This is a competitive grant program, providing more than $42 million to grantees over a four year period. The goal of the grant program is to reduce healthcare costs in the Commonwealth by supporting evidence-based community and clinical prevention strategies and providing activities that will simultaneously decrease preventable risk factors and illness and improve the management of existing chronic disease.
The Department's overall strategy to improve health and reduce healthcare costs is to make a relatively small number of awards to neighborhoods and communities across the state. To enhance the impact, interventions will occur simultaneously in clinical and community settings with required linkage across the two. By limiting the number of awards, we ensure that communities will have sufficient funds to implement these interventions. Each awardee will address at least two of four "priority" health conditions. "Optional" conditions should be included only if they strengthen the overall proposal. The determination of "priority" conditions vs. "optional" conditions was based primarily on the likelihood that effective interventions for "priority" conditions would be more likely to lead to measureable improvements in health and reductions in costs during the grant period. These are the central goals of Chapter 224.
DPH anticipates awarding between six and twelve grants with the intent of achieving an equitable distribution of awards across the Commonwealth.
