VENDOR ACTION REQUIRED: The purpose of this amendment is to alert vendors that on October 29th OSD's Audit Unit will be sending an email notice to the vendor email address on record advising vendors that the Third Quarter (7/1/2010-9/30/2010) Statewide Contractor Administration Fee Reports and Payments are due on 11/15/2010. The required report form is available on the Forms & Terms tab of this Contract record. Vendors on statewide contracts are responsible for timely submission of the report and payment; failure to comply with this contractual requirement can result in contract suspension. Vendors should email questions to
firstname.lastname@example.org for review and routing to the OSD Procurement Manager or Audit Unit.
